Practice Pro-Forma

Dartford, Gravesham and Swanley Health and Care Partnership is working with ECLIPSE to provide a new equality of care model for women’s health across the region. The purpose of this programme is to address the inequalities of care currently present across the Kent and Medway region in relation to the support and provision of HRT for individuals within the deprived and ethnic minority groups. There is currently a 5:1 ratio across Kent and Medway in relation to the provision of HRT between white and ethnic minority individuals. The interface will automatically contact women between the ages of 45-55 to identify any menopausal symptoms and the personal preferences of the individuals in relation to receiving HRT therapy. The Dartford, Gravesham and Swanley Health and Care Partnership is leading on this exciting programme which if successful will be rolled out to the wider Kent and Medway region. 

This programme has already been implemented in other regions with significant improvements in equality of care statistics, and will be utilised to address inequalities across the ICB.
