Project overview:
Lipid Perfect is a programme that forms part of the wider “Your Healthier Future” initiative being offered to practices throughout Cambridgeshire & Peterborough over the next two years. Lipid Perfect is an extension of the CHOL001 programme that was rolled-out earlier this year to assist practices in engaging with patients eligible for statin initiation.
This programme aims to identify and engage with patients eligible for statin therapy, in line with to NICE guidelines, who are currently not on any lipid lowering therapy.
We would like to invite you to complete an expression of interest and provide practice permissions for the Lipid Perfect pathway within Eclipse to enable further programme rollout and patient engagement.
NICE guidance states that high-intensity statins are the most clinically effective option for the secondary prevention of CVD – that is, reducing the risk of future CVD events in people who have already had a CVD event, such as a heart attack or stroke (QS100).
Given the cardioprotective effect of statins, CG181 outlines the indications for utilisation of a statin for primary prevention. This applies to patients with chronic kidney disease, type 1 diabetes or if their QRISK is greater than 10% (CG181).
Objectives
To identify and prioritise patients aged 18 and over, with no GP-coded cardiovascular disease, with a QRISK >20% and not on lipid lowering therapy.
To identify and prioritise patients aged 18 and over, with GP-coded cardiovascular disease and not on lipid lowering therapy.
Reviewing and offering these patients lipid lowering therapy as clinically appropriate.
There are two tiers of support on offer to improve the for eligible patients at your practice:
Eclipse Detect – Activation of this programme will commence the statin optimisation pathway within Eclipse, providing a list of all eligible patients, alongside additional priority groups, which can be further filtered and prioritised by metrics such as levels of deprivation, ethnicity, comorbidities, and risk.
Eclipse Complete – Will then automatically engage with these priority patient groups via the PEGASUS platform to inform, educate, counsel and consent patients to consider statin initiation. A dynamic questionnaire will capture additional risk factors and preferences related to this. Results will be captured within Eclipse and form the basis for an individualised action plan which can be reviewed by the practice prescriber.
The solution being offered utilises Eclipse’s digital patient engagement platform which will automatically identify your eligible patients, engage with them via SMS, educate and counsel them using a lander page and consent them to be initiated on a lipid-lowering therapy.
REMUNERATION:
This patient support programme is part of the Cambridgeshire & Peterborough Your Healthier Future programme. There will be some additional funding for practices participating in this programme. Practices have three choices:
Eclipse Detect - Practices that would like to activate Eclipse Detect to risk stratify and identify patients with a statin indication, will undertake patient engagement and patient recalls through their own practice team. Payment to practice is set at £1,000 if the target* is met.
Eclipse Complete - Practices that would like to activate Eclipse Complete will have all patient engagement undertaken for them through a centralised patient support hub. Payment to practice is set at £400.
No participation - Practices that would not like to utilise either Eclipse Detect or Complete will not receive any additional financial remuneration for the hypertension detect programme.
*Individual practice targets will be based on an average of the proceeding 2 years of practice performance for each of the metrics being targeted in the Your Healthier Future programme. These targets have been approved by the ICB and require an equal effort from all practices within an upper and lower threshold.
EVALUATION: As well as quantitative data, which will be collected via Eclipse, patient reported outcomes and perceptions of the new pathway will be collected through patient questionnaires.
