Practice Pro-Forma

Greater Manchester ICB is offering practices the opportunity to opt-in to the Medicine’s Optimisation dashboard and digital patient engagement platform within Eclipse.

The Eclipse BOOMERANG programme can provide your practice with the opportunity to make significant cost-savings for the region through automated digital engagement for patients eligible for agreed medication optimisations.

The steps involved in this process are:

  1. Practices complete this pro forma to provide permissions for the programme to be implemented at their practice.

  2. Regionally agreed pathways are developed for the medication optimisations and eligible patient groups, to be included onto the Eclipse BOOMERANG dashboard.

  3. Eligible patients are sent a text message, with a link to a patient information lander page, which will explain the proposed medication optimisation including the benefits and any important information to be aware of.

  4. Patients are directed to complete a patient questionnaire within NHSConnect.org to express their preferences and consent for or opt-out of an optimisation.

  5. Patient responses are captured, and cohorts are formed within Eclipse according to the action required.

  6. Practices carry out the appropriate action for the patient (e.g. prescribe the medication, discuss with the patient, or code as offered and declined) and record this within the system for longitudinal outcome tracking and cost-saving evaluation.

All defined medicine optimisations and patient engagement materials will be confirmed by the Greater Manchester ICB Medicines Optimisation Team and appropriate clinical steering group.

This process will:

  • Produce significant cost-savings for the ICB.

  • Improve compliance with regional formulary and preferred medication choice.

  • Reduce workload for practices and prescribers.

  • Improve patient satisfaction.
